This survey seems to be going out to certain EQ2 players and apparently it will be sent out on a semi-periodic basis, perhaps quarterly. I received a copy on each of the e-mail addresses associated with my accounts:

A near pointless survey which will generate absolutely no information to make the game better. All it will serve a purpose for is for the Finance team to turn around and cut staff numbers.
Most likely that is the outcome they are hoping for…a reason to cut more of the staff and ultimately sunset the game. I think the only reason they released that poll is because their bottom line is suffering.
I’d assume they’re making a killing on EQ2, they release two weeks worth of content a year and charge 2/3rds of a fully featured AAA game for it. Plus they try to force people into subscribing on top of that. If games like ESO, Tera, PoE, and Warframe can make money releasing more content for less then I can’t fathom how Daybreak could be struggling.
I filled out the survey and left a comment. Said I hadnt played in acouple of years because of a lack of new content. Also said I will come back when I determine that there is enoungh content to keep I and the wife busy for 6 months or so. I really got tired of doing xpac content in less then a month.
I had a lot to say if I would have received that poll. I still have sub, but its up in 6 months. My wife’s sub has already been canceled and we did not get her the xpac. I got a lot to say about how unfriendly it is for a solo player.

Pretty frequently actually. I’d say at least once a week when I was actively playing it would crash. Which is great because since the game client decides to dim my monitor for some reason, it doesn’t undim it when it crashes! The best part though is that when it crashes it pulls my browser up and begs me for money. It’s pathetic.
